After the tour I strongly suggest you go to the top of the Basilica. This is not included in the tour, but it is worth the money and sore legs. The hardcore visitor will take the stairs all the way to the top - 551 steps - but if you pay a little extra you can take the lift and you are "only" left with 320 steps. It is tough but worth it. I also agree with a previous comment that the Vatican are allowing far too many people in on a daily basis as it was really difficult to navigate through the crowds
